1º.-1. I am not suffering any disease or medical condition that would prevent or restrict sport diving practice, I have been informed of the risks that this activity and specifically states involved in. I have a medical certificate that allows me to practice sport diving and I accept to show the medical certificate to the local authority when ask me from it, that’s why I let all my personal information at the beginning of this document.

2º.- I accept not to dive below my dive certification limit and I also get compromised to not do decompression dives practicing sport diving. I also accept that it is not allowed to dive alone, that I’m responsible about my buddy and my buddy is responsible of me.

3º.- I accept to follow the next dive centre rules:
● The dive time is 60 minute on day dives and 45 minutes on night dives..
● It is not allowed to dive under 50bars pressure in the cylinder.
● I get the compromise to be respectful with the sea environment, to not touch and/or bother any sea life.
